Jackson Care Connect has opened a grant opportunity for projects taking place in 2023. There is a total of $180,000 available for this opportunity, and the maximum grant award for an organization is $35,000.

Applicants must be a nonprofit organization or a government agency, and grant applications must address at least one of the Community Health Improvement Plan (CHIP) priority areas. Those areas are Housing, Behavioral Health and Well-being, Parenting Support and Life Skills, and Health Equity. Applications will be reviewed by our Community Advisory Council (CAC).

Applications are due September 1, 2022. See full details and a link to the Zoom Grant application on the CHIP grant page.
