Since 2012, when coordinated care organizations (CCOs) were first formed, Jackson Care Connect has been dedicated to our mission of empowering people and systems to improve the health of our community. Over the years we have formed strong partnerships
with clinical providers and community partners to help us serve that mission.
We are excited to share the Jackson Care Connect 2023 Report to the Community! The theme for the report is “expanding access to meet community needs.” The report contains stories on JCC’s second annual Recovery Summit, the collaborative effort to produce its updated community health assessment, JCC support for traditional health worker services, and other stories and data points illustrating the CCO’s impact.

The 2021 Report to the Community, Partnering to protect health and improve care, describes the heroic work of our clinical and community partners, as they weathered the second year of the pandemic. See the report below for highlights on our partnerships and collaborations throughout 2021.
The 2020 Report describes how we worked with partners to serve our members and community throughout the challenges of 2020, especially, the COVID-19 pandemic and the Almeda and Obenchain fires. It also describes the strength of our network, which is helping our members and community be healthier every day.
