Since 2012, when coordinated care organizations (CCOs) were first formed, Jackson Care Connect has been dedicated to our mission of empowering people and systems to improve the health of our community. Over the years we have formed strong partnerships
with clinical providers and community partners to help us serve that mission.
We are excited to share the Jackson Care Connect 2024 Report to the Community! The report contains stories on the post-pandemic return of our Spring Conference, the completion of a new five-year Community Health Improvement Plan for Jackson and Josephine counties, the impact of our investments in behavioral health care, and more.
It also contains infographics highlighting our impact in a number of other areas, from member engagement in dental care to provider satisfaction. As usual, we wrap up our yearly report with a listing of the dedicated people who served on our 2024 Board of Directors. Please take a few moments to get to know us better.
Past Reports to the Community
Read the Jackson Care Connect 2023 Report to the Community here. The theme for the report is “expanding access to meet community needs.” The report contains stories on JCC’s second annual Recovery Summit, the collaborative effort to produce its updated community health assessment, JCC support for traditional health worker services, and other stories and data points illustrating the CCO’s impact.

The 2020 Report describes how we worked with partners to serve our members and community throughout the challenges of 2020, especially, the COVID-19 pandemic and the Almeda and Obenchain fires. It also describes the strength of our network, which is helping our members and community be healthier every day.
