Jackson Care Connect is committed to helping you and your family stay well and live a healthy life. You can help prevent many serious illnesses and health conditions by making small, healthy lifestyle changes, using preventive health and wellness services, and working with your primary care provider (PCP).

Jackson Care Connect's care coordination services

Our care coordination services are available to all Jackson Care Connect members. When you request these services, a care coordinator will help you:

Work with the health care system.
Find primary care and specialty providers.
Access community and social services.
Get services and medical equipment.
Coordinate your care among your providers if you have:

Care coordination services support your whole-person health. That means your physical health, but also your mental and oral health, too. We will create a care plan that is based on your needs, so you can reach your health care goals.

If you have both Medicaid and Medicare, we can also help coordinate your care between them. We want to make sure you get the care and services you need.

No referral is needed for coordinated care services. Call Customer Service at 541-500-0567, toll-free 855-722-8208 or TTY 711. We will put you in touch with a staff member who is specially trained in care coordination.
