In 2023, Jackson Care Connect was one of four lead partners on All in For Health, a partnership between coordinated care organizations and public health departments serving Jackson and Josephine counties. This group was further supported by many organizations including hospitals, community-based organizations, educational services and public health departments. The group worked together to conduct a regional Community Health Assessment (CHA) in Jackson and Josephine counties. The CHA was a year-long process to describe and understand the health of the community of southern Oregon. The CHA helps guide planning and action to improve health in Jackson and Josephine counties.
In addition to research, community members shared their thoughts through interviews, focus groups, data walks and a community-wide health survey.
