How to make a complaint or grievance

If you are unhappy with us, your health care services or your provider, you can complain or file a grievance at any time. We will try to make things better. You can download a complaint form in English or Spanish or ask our Customer Service team for help filing your complaint. You also can ask your caseworker for help or the Public Benefits Hotline (a program of Legal Aid Services of Oregon and the Oregon Law Center) at 800-520-5292, TTY 711, for advice and possible representation.
