The Advanced Illness Care program
The Advanced Illness Care program helps those who are diagnosed with a serious illness. It offers specialized care to help relieve the symptoms and stress of a serious illness.
If you qualify, Housecall Providers can support you over the phone or in your home. There is no charge for eligible patients.
If you decide to use this service, staff will work closely with you and your other health care providers. They will focus on what is most important to you. Their goal is to help you live as well as possible with your illness to improve the quality of your life.
The Advanced Illness Care team
The team includes:
- Nurses
- Social workers
- Community health workers
- Spiritual counselor
Support offered
Pain and symptom management
Expert answers about your illness, medicines and treatment options
Counseling and emotional support
Communication with your primary care provider (PCP) and specialists
Care to help you avoid emergency room visits
Other resources to support you and your care
Extra support for advanced illness
The Housecall Providers Advanced Illness Care program offers an extra layer of support for you and your caregivers.
It will help you live as well as possible with your illness.
Examples of advanced illness:
- Cancer
- Heart disease
- Liver or kidney disease
- Lung conditions such as COPD
- Neurological diseases (affecting the nervous system) such as Parkinson’s or ALS
- Immune diseases such as HIV/AIDS
