Community involvement

Jackson Care Connect partners with clinics, organizations and projects to support the well-being of our members and improve the health of our community.

Jackson County Continuum of Care (CoC)

Jackson Care Connect supports this housing collaborative, which is a community-wide effort to end homelessness. They provide stable housing for those experiencing homelessness and offer preventive services for those who are at risk. While the CoC addresses the housing needs of all homeless residents of Jackson County, it especially targets housing and support service needs of those who are chronically homeless.

Jackson Care Connect partners in this health equity coalition, serving on its steering committee and other workgroups. Our involvement in workgroups on equity, diversity and inclusion and reproductive and sexual health has given us the chance to promote health equity and reduce health disparities by working with organizations across all sectors.

Southern Oregon Early Learning Hub

Early childhood education has a strong impact on lifelong health. Jackson Care Connect partners with the Southern Oregon Early Learning Hub on shared initiatives to improve health outcomes and learning opportunities for children ages 0-5. The Early Learning Hub builds strong partnerships to create effective programs that promote school readiness and family stability.

Southern Oregon Success

Jackson Care Connect supports efforts to raise awareness of the effects of adverse childhood experiences (ACEs), resilience and self-regulation. They offer training sessions for educators and community groups. We also support Southern Oregon Success’s efforts to integrate this work into major community initiatives, like the Community Health Improvement Plan.

Stay Safe Oregon

Jackson Care Connect is part of Stay Safe Oregon, which delivers information and support to patients and families coping with the opioid crisis. Several coordinated care organizations — along with county-run alcohol and drug education programs — met with KOBI-TV leaders and crafted a campaign that combines public service messages on TV with the Stay Safe Oregon website.
