Welcome to our 2025 Report to the Community! This report covers a year of fast-paced change and new challenges, including federal policy shifts impacting Medicaid and other social-safety-net programs. Through it all, Jackson Care Connect (JCC) held fast to our mission of empowering people and systems to improve the health of our community. We’re proud of what we accomplished as a result.  

JCC has been one of the few CCOs in Oregon to earn 100% of the state’s Quality Incentive Program payout for the past 11 years. Our strong performance on health care quality measures is closely tied to the deep partnerships we’ve built with providers in our network. We’ve also prioritized supporting our network in many other ways, including learning opportunities, such as the new health care interpreter summit featured in this report.

Supporting children and families has been another top priority. It’s not only the right thing to do but a reflection of who we serve. Nearly two in five of our members are under the age of 18. Many of the programs highlighted in this report have improved access to care for children and families and other groups, including members who are unhoused and have medical needs.  

While we continue to navigate uncertainty in the Medicaid space and beyond, we remain committed to our members, our clinical and community partners and quality health care. We continue to innovate, strengthen partnerships and reinvest locally to ensure our members get the whole-person care they deserve.
