Jackson Care Connect accepts applications for Community Health Improvement Plan grants

MEDFORD, Ore. — Local organizations focused on the community’s health now have a funding opportunity through Jackson Care Connect’s 2018 Community Health Improvement Plan (CHIP) grants.

The CHIP identifies strategies to support the health of individuals and the community. One of the goals of the CHIP is to support community partners who are doing service work to address the priorities of Healthy Beginnings, Healthy Living and Health Equity. Jackson Care Connect’s 2018 grants will fund projects linked to these priorities.

As a non-profit, community-focused Coordinated Care Organization providing care for Oregon Health Plan (Medicaid) members in Jackson County, Jackson Care Connect is committed to investing in the community’s health. The Community Advisory Council, which is made up of Jackson Care Connect members and other community stakeholders, will oversee the selection of the CHIP grants.

Proposals for CHIP grants will be accepted from March 15 – April 27, 2018. Grant awardees will be notified by June 1, 2018.

Any nonprofit or governmental organization that serves Jackson County is eligible to apply. Organizations will be required to show proof of non-profit status. CHIP grant guidelines and applications requirements are available online at jacksoncareconnect.org.
