Jackson Care Connect coordinates physical, behavioral and oral health care for Oregon Health Plan (Medicaid) members in Jackson County. We care for the whole person, making it easy to connect to provider groups, hospitals, agencies and community organizations who work together with our member. Jackson Care Connect is part of CareOregon, a nonprofit community organization based in Portland, Oregon. Since 1994, CareOregon has been providing health plan services to Oregonians and developing innovative approaches to care.

REGIONAL CARE TEAM
Care Coordination at Jackson Care Connect is provided through our Regional Care Team (RCT). The regional care team is made up of clinical and non-clinical staff that work directly with members, providers, and community.
Behavioral Health Care Coordinator
In conjunction with the above description of “care coordination” the Behavioral Health Care Coordinator (BHCC) is responsible for developing and implementing member-centric, individualized care plans and providing telephonic care coordination for members with complex or special health care needs and who are identified as high-risk due to behavioral health issues. Registered Nurse – Care Coordinator
In conjunction with the above description of “care coordination” the Nurse Care Coordinator is responsible for developing and implementing member-centric, individualized care plans and providing telephonic care coordination for members with complex or special health care needs and who are identified as high-risk due to medical issues. Intensive Care Coordinator
The Intensive Care Coordinator (ICC) is responsible for developing and implementing member-centric, individualized care plans and providing telephonic and community-based care coordination for the most vulnerable members, including members with complex behavioral concerns, severe and persistent mental illness, and substance use disorders. The ICC utilizes clinical expertise in behavioral health conditions and knowledge regarding the adult and children’s system of care to provide coordination for members as they transition in and out of intensive community and facility based psychiatric treatment settings. Director, Clinical Integration
The Director of Clinical Integration is responsible for directing the integration and execution of network and clinical strategy for Jackson Care Connect. Primary duties include operational planning and oversight, as well as resource, relationship, and people management. This position provides input into strategic plans for the broader organization. This position operates as a dyad with the CCO Medical Director, assuring financial sustainability and integration of CCO-funded programs, including Quality Pool grants, community risk share funds, APMs and/or other CCO-derived investments for clinical performance. The position is a part of the leadership matrix within the CCO and CareOregon to meet CCO strategic initiatives and mission. Learn more and apply.
The Panel Coordinator provides complex and diversified support to CareOregon patients within the context of a primary care medical home, from a team approach, and in continuous partnership with patients and physicians to promote: timely access to needed care, comprehension and continuity of care, and the enhancement of patient well-being within all models of care from traditional to non-traditional. This position provides technical assistance, coaching, and training to the JCC provider network in areas related to mental health and substance use health services. This position spends time designing and facilitating collaborative meetings, and providing on-site support to member mental health, and substance use treatment organization leadership and care teams to implement systems, processes, and workflows that improve quality outcomes, patient experience, clinic and team satisfaction, and reduced health care costs.
